Large Korean Study Links a Simple Marker of Cellular Health to Hearing Thresholds After Age 40

In more than 5,400 adults, each one-degree increase in bioelectrical phase angle, a painless measure of cellular health, was tied to measurably better hearing thresholds and lower odds of hearing loss.

Hearing has long been treated as a sense that ages on its own schedule, separate from the rest of the body. But a growing line of research suggests the opposite: how well we hear in later life appears to track with how well the body as a whole is holding up.

A new population-based study from South Korea adds a striking data point to that picture. Using a quick, non-invasive body composition measurement already common in clinics and gyms, researchers found a consistent relationship between cellular health and hearing ability in adults over 40.

Background: Why the Researchers Looked at This

Bioelectrical phase angle is a number produced during bioelectrical impedance analysis, the same painless test many body composition scales use, in which a tiny electrical current is passed through the body. The phase angle reflects how intact and healthy cell membranes are, along with fluid balance, nutritional status, and what researchers call physiological reserve, the body's overall capacity to withstand stress. Higher values generally indicate healthier cells.

Phase angle has already been linked to outcomes like frailty, malnutrition, and survival in various illnesses. What had not been clearly established is whether this whole-body marker also relates to objective sensory function, such as hearing measured by audiometry, in later life.

The Ewha Womans University team set out to test that question in a large, nationally representative sample, asking whether adults with lower phase angles also tend to have worse hearing once age and other health factors are taken into account.

How the Study Was Done

The researchers analyzed data from the 2022 to 2023 Korea National Health and Nutrition Examination Survey, a recurring government health survey of the Korean population. They focused on adults aged 40 and older, yielding 5,436 participants.

Each participant's whole-body phase angle was measured by bioelectrical impedance analysis at a frequency of 50 kilohertz. Hearing was assessed with pure-tone audiometry, the standard beep test performed in a booth, from which the team calculated each person's better-ear average across speech frequencies as well as high-frequency thresholds. The analyses were adjusted for age, sex, body mass index, high blood pressure, diabetes, current smoking, and alcohol use, so the phase angle relationship would not simply reflect those factors.

What the Researchers Found

The relationship was consistent across every way the team measured it. Each one-degree increase in phase angle was associated with a 1.50-decibel lower, meaning better, speech-frequency hearing threshold in the better ear, and a 1.51-decibel lower high-frequency threshold.

Higher phase angle also meant lower odds of crossing the clinical line into hearing loss, defined as a better-ear average of 25 decibels or worse. For each one-degree increase in phase angle, the odds of audiometric hearing loss fell by about 21 percent.

Notably, when the researchers looked at younger participants aged 12 to 39 in an exploratory analysis, the association was not there. The link between cellular health and hearing emerged only in adults 40 and older, exactly the group in which age-related hearing decline begins to accumulate.

The authors are careful to frame phase angle as a complementary research marker for now, one that links objective sensory function with the body's systemic reserve, and they note that longitudinal validation is required before it could be used for any kind of screening.

What It Means for People with Hearing Loss

The practical message of this study is not that anyone should rush out and measure their phase angle. It is that hearing appears to be woven into overall health in midlife and beyond. The same bodily reserve that shows up in a cellular health marker also shows up, in a small but measurable way, in the audiogram.

That reframing matters for behavior. If hearing is part of general health rather than an isolated sense, then it deserves the same routine attention we give blood pressure or weight: periodic checks after 40, and action when something has changed, rather than the years of delay that are typical between first noticing hearing difficulty and doing anything about it.

Treating Hearing as Part of Routine Health Upkeep

Because this study frames hearing as something to monitor alongside the rest of your health after 40, it is worth knowing that acting on a hearing change no longer requires a long clinical pipeline. Limitations of This Research

This was a cross-sectional study, a snapshot in time, so it cannot show that declining cellular health causes hearing loss, or in which direction the relationship runs. Both could be downstream of some shared aging process. The effect sizes are also modest at the individual level: a decibel and a half per degree of phase angle is meaningful across a population but would not be noticeable to any single listener.

The sample was drawn entirely from South Korea, so replication in other populations is needed, and as the authors themselves stress, longitudinal studies must confirm the association before phase angle could play any role in hearing screening.

What to Do With This

Think of this study as one more thread stitching hearing into the fabric of general health. The habits that protect the body as a whole, staying active, eating well, managing blood pressure and diabetes, may be quietly relevant to the ears too. And after 40, a periodic hearing check deserves a place on the same list as the annual physical, because changes caught early leave the most room to act.
