Can Eating More Fish Protect Your Hearing? A Large Japanese Study Says Probably Not

A study of 13,908 Japanese adults found no link between seafood or omega-3 intake and better hearing, challenging a popular idea about preventing age-related hearing loss.

Age-related hearing loss is one of the most common conditions of later life, and it carries weight far beyond the ears. It has been associated with dementia, social isolation, and reduced quality of life, which is why researchers keep looking for ways to slow it down before it starts.

One appealing candidate has long been diet, and specifically the omega-3 fatty acids found in fish. The theory is plausible: these fats have anti-inflammatory and antioxidant effects that might protect the delicate structures of the inner ear. A team at Tohoku University in Japan put that theory to an unusually strong test, in a country where people eat more seafood than almost anywhere else.

Background: Why the Researchers Looked at This

Omega-3 polyunsaturated fatty acids, often shortened to n-3 PUFAs, are the fats found abundantly in oily fish like salmon, mackerel, and sardines. Because oxidative stress and inflammation are thought to contribute to the gradual decline of hearing with age, nutrients that counter those processes have been proposed as protective.

Earlier studies on fish intake and hearing produced inconsistent results, with some suggesting benefit and others finding none. Many of those studies were conducted in Western populations where fish consumption is relatively low, making it hard to detect differences. Japan offered the researchers a natural advantage: a population with high and widely varying seafood intake, plus a large biobank project with careful health measurements.

How the Study Was Done

The researchers analyzed 13,908 adults aged 50 to 79 from the Tohoku Medical Megabank Project Cohort Study. Each participant's hearing was measured with pure-tone audiometry, the standard clinical hearing test, at four frequencies spanning the range most important for speech: 500, 1,000, 2,000, and 4,000 Hz.

Diet was assessed with a validated food frequency questionnaire, and intakes of seafood, omega-3 fats, and saturated fat were each divided into quartiles, from the lowest quarter of consumers to the highest. Statistical models then compared hearing thresholds across those quartiles while adjusting for other factors, with men and women analyzed separately.

What the Researchers Found

The headline result was a null one. Neither seafood intake nor omega-3 fatty acid intake was significantly associated with hearing thresholds in men or in women. Even in a population eating far more fish than most Western cohorts, the people who ate the most showed no measurably better hearing than the people who ate the least.

The one signal that did emerge was unexpected. In men, higher saturated fat intake was associated with slightly better hearing at 1,000 Hz, a difference of about 2.2 decibels between the top and bottom quartiles, and about 1.7 decibels on the pure-tone average across frequencies. Lower saturated fat intake, in other words, tracked with slightly worse hearing in men.

In women, no dietary measure showed any significant relationship with hearing at all. The authors conclude that the relationship between dietary fats and age-related hearing loss is likely complex and may differ between men and women, and they stop well short of recommending any dietary change based on these findings.

What It Means for People with Hearing Loss

For anyone who has been eating fish partly in hopes of protecting their hearing, this study is a useful reality check. It does not say fish is bad, and seafood remains a well-supported part of a heart-healthy diet. It simply suggests that omega-3 intake is unlikely to be a reliable shield against age-related hearing decline.

The broader lesson is about where to place your effort. Age-related hearing loss develops gradually and affects a large share of adults over 65 regardless of lifestyle. Since no dietary silver bullet has held up under scrutiny, the practical priorities remain the proven ones: protecting your ears from loud noise, getting your hearing checked, and addressing hearing loss directly when it arrives rather than waiting years, as most people do.

If Prevention Has Limits, Acting Early Matters More

Because this study suggests diet cannot be counted on to hold off age-related hearing loss, the question shifts from how to prevent it to how easily people can act once it appears. Limitations of This Research

This was a cross-sectional study, meaning diet and hearing were measured at the same point in time, so it cannot show how eating patterns over decades relate to hearing decline. Food frequency questionnaires also rely on memory and self-report. And because participants all came from one region of Japan, the findings may not transfer directly to populations with different diets or genetics. The surprising saturated fat result in men, in particular, needs confirmation in other cohorts before anyone reads much into it.

Where This Leaves Us

A large, well-measured study in an ideal population for the question found that fish and omega-3 intake do not appear to protect hearing as it ages. Diet still matters enormously for overall health, but for hearing specifically, the evidence keeps pointing toward the same practical advice: guard your ears from noise, test your hearing regularly, and act promptly when hearing starts to fade.
