Worse Hearing Tied to More Brain Amyloid in Late Middle Age, PET Imaging Study Finds

In a cohort of 223 late-middle-aged adults, worse hearing on standard tests was associated with higher levels of amyloid, the protein that accumulates in Alzheimer disease, across several brain regions on PET scans.

The connection between hearing loss and dementia has become one of the most closely watched questions in aging research. Large epidemiological studies have repeatedly found that people with hearing loss face a higher risk of cognitive decline, but the biological pathway behind that link remains unsettled.

A new imaging study from researchers at Columbia University approaches the question from a different angle: instead of tracking memory test scores over years, it looks directly at amyloid, a hallmark protein of Alzheimer disease, in the brains of adults in their early to mid sixties.

Background: Why the Researchers Looked at This

Amyloid beta is a protein fragment that can clump into plaques in the brain, one of the defining features of Alzheimer disease. Researchers can now measure it in living people using positron emission tomography, or PET, which produces a number called the standardized uptake value ratio, or SUVR. A higher SUVR means more amyloid.

Hearing was measured two ways. Pure-tone average, or PTA, captures the quietest tones a person can detect, with higher decibel values meaning worse hearing. Word recognition score, or WRS, measures the percentage of spoken words a person can correctly repeat, which reflects clarity of hearing rather than just volume.

If hearing loss were linked to amyloid buildup, that would point toward a shared biological pathway between hearing and Alzheimer disease. If not, the hearing and dementia connection might run through other routes, such as social isolation or reduced stimulation. Either answer matters for how clinicians think about midlife hearing care.

How the Study Was Done

The team studied 223 participants with an average age of 65.6 years. Each underwent hearing testing in the better ear, measuring both PTA and WRS, and each received an amyloid PET scan. The average PTA was 21.0 decibels, which sits right at the border of what audiologists classify as mild hearing loss.

The researchers then used multivariable linear regression, adjusting for relevant covariates, to test whether worse hearing was associated with higher amyloid, both across the whole brain and in specific regions such as the temporal, parietal, cingulate, and frontal areas.

What the Researchers Found

For pure-tone hearing, every 10 decibel worsening in PTA was associated with a 0.024 higher amyloid SUVR in the left temporal region, with a P value of 0.048. Only that region reached statistical significance, but the authors note that the associations were similar in size and direction across all brain regions examined.

The word recognition findings were broader. For every 10 percent drop in WRS, amyloid was significantly higher in the left temporal, right temporal, left parietal, right parietal, left cingulate, right cingulate, and right frontal regions, as well as globally across the brain, where the coefficient was 0.064 with a P value of 0.005.

In other words, the clarity measure, how well people understood words, showed a more consistent relationship with brain amyloid than the volume measure. The authors observed that the associations appeared to be driven by participants with the greatest hearing loss and amyloid burden.

The effect sizes were small, and the authors are careful to frame the results as an association that requires further investigation, including the open question of whether hearing could be causally related to amyloid plaque at all.

What It Means for People with Hearing Loss

This study does not show that hearing loss causes Alzheimer disease, and it does not show that treating hearing loss prevents it. What it adds is a piece of biological evidence that hearing and brain health are intertwined earlier than many people assume, in a cohort whose average hearing loss was only mild.

The practical message is about attention rather than alarm. Hearing changes in your fifties and sixties are common, measurable, and manageable. Given the accumulating evidence connecting hearing to brain health, a hearing check in late middle age is a low-cost, low-risk step, particularly for people who notice they can hear speech but struggle to make out the words.

A notable detail in this study is that word recognition, not just tone detection, showed the strongest link to brain measures. That echoes what many people with early hearing loss describe: the television is loud enough, but dialogue is muddy. Limitations of This Research

This was an observational study at a single point in time, so it cannot establish cause and effect. The statistically significant PTA finding was limited to one brain region and sat near the conventional significance threshold, and the effect sizes throughout were small. The cohort was drawn from a single medical center population, which may limit generalizability. The authors themselves emphasize that the reason for these associations, and whether hearing could be causally related to amyloid, requires further investigation.

Where This Leaves Us

The hearing and brain health story is still being written, and this study adds a careful, imaging-based chapter rather than a conclusion. What it reinforces is that hearing in late middle age is not a peripheral concern. It is measurable, it tracks with what is happening in the brain, and it deserves the same routine attention as blood pressure or vision.
