Nearly 16 Million People Live with Hearing Loss in Ethiopia: A 33-Year Analysis Highlights the Global Hearing Care Gap

A new analysis of Global Burden of Disease data finds that hearing loss affected an estimated 15.93 million people in Ethiopia in 2023, with mild hearing loss dominating the burden and researchers calling for expanded access to hearing care.

Hearing loss is often described as an invisible epidemic. It rarely makes headlines, yet it consistently ranks among the leading causes of years lived with disability worldwide. Understanding where the burden falls, and how it is changing, is essential for health systems deciding where to invest.

A newly published analysis takes a detailed look at one of Africa's most populous countries, tracking the prevalence of hearing loss in Ethiopia across more than three decades, from 1990 to 2023.

Background: Why the Researchers Looked at This

The Global Burden of Disease (GBD) study is a massive, ongoing international effort that estimates the impact of hundreds of diseases and injuries in every country, using standardized methods so results can be compared across time and place. One of its key measures is years lived with disability, or YLDs, which captures not whether a condition kills, but how much healthy life it erodes.

Hearing impairment is increasingly recognized as a major public health issue, linked in other research to social isolation and reduced quality of life. Yet country-level data for many low- and middle-income nations remain sparse. The authors set out to quantify how common hearing loss is in Ethiopia, how much disability it causes, and how both have shifted since 1990.

How the Study Was Done

The researchers drew on the GBD 2023 estimates, which combine survey data, health records and statistical modelling to produce nationally representative figures. For hearing loss, the GBD framework classifies severity into categories ranging from mild through moderate, moderately severe, severe, and complete loss.

The team extracted prevalence counts, age-standardized rates per 100,000 population, and YLDs for Ethiopia for each year from 1990 to 2023, with uncertainty intervals around every estimate. Age-standardization allows fair comparisons over time even as a country's population grows and ages.

What the Researchers Found

In 2023, an estimated 15.93 million Ethiopians were living with hearing loss, an age-standardized prevalence of about 21,702 per 100,000 people, which is roughly one in five when standardized for age. The condition accounted for an estimated 580,148 years lived with disability in that year alone.

The burden was overwhelmingly driven by mild hearing loss, which accounted for about 10.6 million of the cases. Both prevalence and disability rose steadily with age, consistent with age-related hearing loss being a primary driver.

The trends over 33 years tell a nuanced story. Overall age-standardized prevalence rose slowly, by about 3 percent between 1990 and 2023. But the severest forms declined markedly: rates of complete hearing loss fell by 23 percent and severe hearing loss by 3 percent, while mild, moderate, and moderately severe categories edged upward.

The authors conclude that Ethiopia faces a substantial and persistent burden of hearing loss, and they call for integrated screening, expanded access to hearing care, and strengthened health systems to address it.

What It Means for People with Hearing Loss

Perhaps the most striking implication is the sheer dominance of mild hearing loss. Mild loss is exactly the stage where intervention is simplest and most effective, yet it is also the stage most likely to go unaddressed, because people adapt, dismiss it as ageing, or face barriers of cost and distance to clinical care.

The pattern seen in Ethiopia mirrors the global picture: as populations age, mild and moderate hearing loss is becoming steadily more common, and traditional clinic-based models alone cannot reach everyone who could benefit from screening and amplification.

Closing the Access Gap for Mild and Moderate Hearing Loss

Because this study highlights access to hearing care as the central challenge, it is worth noting how device categories have evolved to lower that barrier. In the United States, FDA-OTC hearing aids were created specifically for adults with the mild to moderate hearing loss that dominates burden estimates like these, removing the requirement for clinic visits and prescription fittings that keep many people from acting on early hearing difficulty.

Limitations of This Research

GBD estimates are statistical models, not direct national surveys, and for countries with limited primary data the uncertainty intervals are wide, as the figures here reflect. The analysis also describes burden rather than causes, so it cannot say why severe forms declined while milder forms rose, though improved treatment of ear infections and infectious disease is a plausible contributor.

Where This Leaves Us

Three decades of data point to a clear conclusion: the future of hearing loss, in Ethiopia and globally, is largely a story of mild and moderate impairment in ageing populations. That is a burden health systems can actually do something about, provided screening and affordable amplification reach the people who need them.
