Hearing Loss Ranks Among the Top Three Occupational Disease Burdens in China, New Analysis Finds

A three-decade analysis of national health data finds that age-related and other hearing loss, together with chronic lung disease and low back pain, accounts for nearly 60 percent of the disease burden linked to six occupational risk factors in China.

When people think of workplace health hazards, they tend to picture dramatic injuries or toxic chemical exposures. Hearing damage rarely makes the list, partly because it accumulates silently over years of exposure to loud machinery, traffic, tools, and industrial noise, and partly because its effects surface late, often after retirement.

A new study published in the Journal of Occupational and Environmental Medicine puts numbers on that hidden toll. Drawing on the Global Burden of Disease database, researchers at the Tianjin Centers for Disease Control and Prevention tracked how much healthy life China has lost to occupational risk factors since 1990, and projected the trend out to 2035.

Background: Why the Researchers Looked at This

Public health researchers measure the impact of disease using disability-adjusted life years, or DALYs. One DALY represents one year of healthy life lost, whether to premature death or to living with disability. The measure allows very different conditions, from lung disease to hearing loss, to be compared on a single scale.

China's workforce is enormous and aging, and its industrial economy exposes millions of workers to noise, dust, fumes, and physically demanding labor. The research team wanted to know whether decades of occupational health regulation had actually reduced the resulting disease burden, which conditions still dominate, and what the next decade is likely to bring.

How the Study Was Done

The researchers drew their data from the Global Burden of Disease Study 2021, a large international effort that produces standardized estimates of death and disability for hundreds of conditions across nearly every country. They focused on six occupational risk factors tracked in that database and the diseases attributed to them.

To analyze trends, they used age-standardized rates, which strip out the effect of a population simply getting older, along with age-period-cohort analysis, a statistical method that separates the effects of age, calendar time, and generation. Finally, they applied Bayesian age-period-cohort models to project the burden forward to 2035.

What the Researchers Found

In 2021, the six occupational risk factors together accounted for 17.90 million disability-adjusted life years in China. That is nearly 18 million years of healthy life lost in a single year to workplace exposures.

The good news is the long-term trend. The age-standardized rate of occupational disease burden fell by 58.8 percent, from 2,362.18 per 100,000 population in 1990 to 974.00 per 100,000 in 2021, reflecting decades of improved workplace safety and health regulation.

The composition of the remaining burden is striking. Just three conditions, chronic obstructive pulmonary disease, low back pain, and age-related and other hearing loss, accounted for 59.5 percent of the total. Hearing loss, in other words, stands alongside chronic lung disease and back pain as one of the three dominant ways work damages long-term health.

Looking ahead, the projections were not uniformly reassuring. The burden attributable to occupational asthmagens and carcinogens is projected to increase through 2035, particularly among women, leading the authors to call for targeted strategies for chronic conditions and vulnerable populations.

What It Means for People with Hearing Loss

The finding that hearing loss ranks among the top three occupational disease burdens confirms what audiologists have long observed: noise exposure at work is one of the most common preventable causes of adult hearing loss, and its effects compound the natural decline that comes with age. Unlike an injury, noise damage is permanent, painless while it happens, and typically noticed only years later, when conversations in restaurants or family gatherings become hard to follow.

The practical lessons run in two directions. For current workers, hearing protection and noise limits genuinely matter, as the 58.8 percent decline shows that regulation works. For the millions of adults already living with the accumulated result, the burden captured in this study is not fixed: much of the disability attributed to hearing loss reflects loss that goes unaddressed, often for years, because of cost, inconvenience, or the belief that nothing can be done.

Closing the Gap for Adults Already Living With Hearing Loss

Because studies like this one measure the burden of hearing loss that is largely untreated, lowering the barriers to treatment is one of the clearest ways to shrink it. Limitations of This Research

Burden-of-disease studies rely on modeled estimates rather than direct measurement, and the quality of the underlying surveillance data varies across regions and decades. Attributing a share of hearing loss specifically to occupational noise involves assumptions that cannot be fully verified, and projections to 2035 depend on trends continuing as modeled. Where This Leaves Us

China's experience shows that occupational disease burden can be cut dramatically, but hearing loss remains stubbornly near the top of what is left. Protecting hearing on the job and addressing hearing loss once it exists are complementary halves of the same public health task, and both are more achievable today than they were a decade ago.
